About Community First Services and Supports (CFSS)

Community First Services and Supports (CFSS) is a new self-directed service that offers participants living in the community who need help with day-to-day activities more control and choice over their care. CFSS replaces Personal Care Assistant (PCA) Choice and Consumer Support Grant (CSG). MNMCHS provides CFSS under the budget model, allowing participants to select their own support workers.

CFSS participants choose a Financial Management Services (FMS) provider to support them with employer-related responsibilities, financial tasks, and budget management. MNMCHS is proud to be an approved FMS provider and is ready to partner with you .

Eligibility for CFSS

Community First Services and Supports requires that a person lives in their own home, be able to direct care or have a representative who can direct care on their behalf, be enrolled in one of Minnesota’s health care programs, and have an assessment that determines they are eligible.

Personal Care Assistance (PCA) Services

At MNMCHS, our PCA services help individuals remain safe, independent, and comfortable in their homes and communities. Our trained caregivers provide personalized support with daily activities based on each person's unique needs and care plan.

Our PCA services may include:
  • Personal hygiene and grooming assistance
  • Bathing and dressing support
  • Mobility and transfer assistance
  • Meal preparation and eating support
  • Medication reminders
  • Light housekeeping related to care needs
  • Community participation and appointment assistance
